National PPO Health Plans

As a member of the ICF, you and/or your employees are eligible to access many exciting plans.

Our National PPO Health Plans offer a variety of deductibles and are available in all lower 48 states.

There are no referrals required and members can go out-of-network to any doctor.

Please contact our office for all health plan benefit summaries and applicable rates.

For coverage to be effective on the first of next month, our office requires completed applications be received by the 20th of the month prior.
